Key Responsibilities:
Conduct patient rounds and manage chronic kidney disease care
Develop and implement individualized treatment plans
Prescribe and manage medications within clinical guidelines
Perform patient evaluations and provide clinical interventions
Educate patients and families on treatment options and disease management
Maintain accurate and timely electronic medical records
Collaborate with physicians and interdisciplinary care teams
Participate in quality improvement and care optimization initiatives
Qualifications:
Active Nurse Practitioner license in California
Master's degree in Nursing
Strong clinical judgment and patient management skills
Effective communication and teamwork abilities
Ability to work in a multidisciplinary healthcare setting
