Job Title: Care Coordination Social Worker IIJob Description
The Care Coordination Social Worker II supports patients and families by completing comprehensive assessments, providing crisis intervention, coordinating care, and connecting members with community resources. This role works closely with healthcare providers, community agencies, and internal teams to ensure members receive the support they need. Accurate documentation, case presentations, and strong advocacy are essential parts of the position.
This is a hybrid role with limited onsite time in the Santa Barbara office. Local candidates (Ventura, SB, SLO counties) are preferred, but candidates anywhere in CA will be considered.
Responsibilities
Complete comprehensive health assessments
Provide crisis intervention and care coordination
Collaborate with community agencies and support programs
Maintain accurate documentation and case records
Present cases at multi/inter-disciplinary complex case conferences
Advocate for member rights, services, and resource access
Required Qualifications
Bilingual: English + Spanish (required)
Experience with:
Comprehensive health assessments
Crisis intervention
Care coordination
Community agency collaboration
Reporting protocols
Presenting at complex case conferences
Master's degree in Social Work, Clinical Psychology, Psychology, or related field
5 years clinical experience (LCSW may substitute for 2 years)
Strong communication, organization, and documentation skills
Work Environment & Details
Hybrid: SB office 1-2 times per month
If outside the area (but in CA): 1x per quarter
Contract: 2/24/26 - 4/24/26
Pay: $40-$43/hr

Pay and Benefits
The pay range for this position is $39.50 - $39.50/hr.
