Title: LTSS Service Care Coordinator
Location: Remote San Antonio, TX. Not doing member visits, but team building exercises do occur occasionally and training. Issues with tech can be fixed in office as well.
Duration : 3 Months with Contract to Hire
Schedule:8am-5pm CST
Job Description:
The LTSS Service Care Coordinator is responsible for providing telephonic support and care coordination services to members with long-term services and support needs. This role requires strong communication, computer proficiency, and a high level of attention to detail while delivering quality member-focused service.

Required Qualifications (Must Haves)
Excellent customer service skills

Strong computer proficiency, including Microsoft Office
High attention to detail and organizational skills
Experience with telephonic work
Reliable high-speed internet connection

Preferred Qualifications (Nice to Haves)
Bilingual (English/Spanish preferred)

Disqualifiers
No prior experience working with Medicaid and individuals with special health care needs

No strict performance metrics
Strong work ethic, time management, and organizational skills required
Productivity log maintained; tasks are fluid and may vary daily

Computer Skills: High level of experience (Microsoft Office required)
