The Windsor Center is a short-stay crisis stabilization unit (CSU) that offers 24/7 services to adults and youth. The Windsor Center aims to increase access to crisis services, reduce inpatient hospitalization, reduce the amount of time that law enforcement is involved in a mental health crisis, and strengthen the existing outpatient behavioral health services. Windsor Center is under contract with the San Bernardino County Department of Behavioral Health.

What You Will Do to Change Lives

Under direct supervision, serves as a primary support and case manager to provide screening, assessment, and assist in linking services needed by members served and their family to overcome the crisis which brought them into the program.

We pay differentials!!! The differential rates are paid at six percent (6%) of an employee's base rate of pay for the evening (PM) shift, and ten percent (10%) of the base rate of pay for the night shift (NOC). Weekend differential rates are paid at five percent (5%) of an employee's base rate of pay for the day (AM) shift, eleven percent (11%) of the base rate of pay for the evening (PM) shift, and fifteen percent (15%) of an employee's base rate of pay for the night (NOC) shift.

Shifts Available: On Call; Shift Hours and Days vary as needed.

Expected starting wage range is $21.00 - $25.15. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

ESSENTIAL FUNCTION

1. Demonstrates the Telecare mission, purpose, values, and beliefs in everyday language and contact with internal and external stakeholders.
2. Collaborates with members served to create Recovery Plan goals and ensure all services described in the Recovery Plan are provided.
3. Consistent with area of expertise, provides specialized services and interventions related to Crisis Intervention, including screening and assessments.
4. Facilitates groups according to the needs and interests of members served especially regarding education and coping skills in the area of Crisis Intervention.
5. Provides appropriate interventions in crisis situations.
6. On a rotating basis, performs officer of the day functions for both wellness, duty and stand by duty tasks.
7. Collects required data and completes required documentation.
8. Coordinates transitions to lower levels of care.
9. Maintains awareness of community resources and assists members served in appropriate use of community resources.
10. Assists members served with all needs, including Activities of Daily Living.
11. Assists members served in making and getting to essential appointments.
12. Assists and advocates for members served in obtaining appropriate benefits; provide representative payee services.
13. Ensures that members served understand their legal status and rights.
14. Helps members served achieve greater independence and autonomy.
15. Safeguards confidentiality; adheres to HIPAA policies.
16. Provides wraparound services that encompass whole person care.
17. Provides transportation when needed and appropriate and linkage to community resources.
18. Must evade members served in the event of assaultive behavior and pass assault crisis/crisis prevention training.
19. Provide and document quality and quantity of direct services as required by program standards.

Duties and responsibilities may be added, deleted, and/or changed at the discretion of management.

Position requires transporting Members in own personal vehicle (acceptable driving record and vehicle inspection for safety).

QUALIFICATIONS

Required:

1. High School Graduation or G.E.D. equivalent and three (3) years of direct services in mental health or community services; or Associate's degree and two (2) years of experience; or Bachelor's degree in Social Services.
2. Must possess at least 1 year of direct service experience with Crisis Intervention, including screening and assessments.
3. Sensitivity to multicultural populations and issues.
4. Must be at least 18 years of age.
5. Must be CPR, Crisis Prevention Institute (CPI), and First Aid certified on the date of employment or within 60 days of employment and maintain current certification throughout employment.
6. Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
7. Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
8. All opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification. Position requires driving, valid driver license, a motor vehicle clearance, and proof of auto insurance is required at time of employment and must be maintained throughout employment. Additional regulatory, contractual or local requirements may apply.

Preferred:

1. Four (4) years' experience in a behavioral health setting.
2. Experience providing intensive case management and/or intensive community supports using psychiatric rehabilitation concepts.
3. Experience in a mental health setting, outpatient clinic, dual diagnosis treatment, or community mental health service.

SKILLS

1. Thorough understanding of the rights of members served and relevant legal processes.
2. Effective written and verbal communication skills.
3. Good computer/technology skills.
4. Ability to assess safety risks.
5. Ability to easily build rapport, maintain engagement with members served, and hold hope in the face of adversity.
6. Excellent driving skills.
7. Knowledge of basic medical and psychiatric terminology.
8. Within two (2) months of employment, ability to apply financial requirements for billing and learn billing codes.

PHYSICAL DEMANDS

The physical demands here are representative of those that must be met by an employee to successfully perform the essential functions of this job. The employee is occasionally required to stand, twist, squat, kneel, push, pull, lift, and carry items weighing 25 pounds or less as well as to frequently sit, walk, reach, bend, and do simple and power grasping. The position requires driving and frequent hazardous exposure (blood borne pathogens, hospital waste, chemicals, and infections).
